Our Brands
The assessor will complete behavioral health assessments and reassessments for children, adolescents and adults. The Assessor will also complete an Individual Recovery Plan along with each assessment/reassessment. This position may be part or full time and the role may be served in conjunction with another position.
The assessor will ensure consumers who enter services receive comprehensive assessments and participate in the development of person-centered Individual Recovery Plans. Assessment information is relayed to the state and appropriate insurance companies to seek out appropriate authorizations.
- Complete assessments, reassessments and Individual Recovery Plans.
- Document contact notes, using a computer-based reporting system, i.e. Care Logic
- Actively participate in team meetings with the Program Director or Clinical Supervisor.
- Attend monthly Clinical Supervision group.
- Actively and timely respond to consumer crisis and supports crisis intervention efforts when BHL is called.
- Staff case with outside agencies as appropriate, i.e., Probation, DFCS, DJJ, CME, LIPT, School, Court system, etc.
- Maintain a caseload size which will allow for quality services, and which will also meet productivity targets.
- Participate in community collaborative meetings.
- Ensure that IRPs are person-centered and meet State Guidelines.
- Provide training on intake process and reassessment process.
- Complete IRP updates, as needed by Program Director or Clinical Supervisor.
- Licensure as a Professional Counselor, Clinical Social Worker, Marriage and Family Therapist or Psychologist is preferred. Candidates who qualify for application for licensure and are designated as a master's level, Mental Health Professional will be considered.
